Services and rates
AG Medical Wellness monthly memberships include the following services:
Primary care by an experienced board certified physician.
- Annual physicals
- Well checks: in office or virtual visits.
- Unhurried on time appointments.
- Exclusive direct communication with your personal doctor via secure text, email or phone.
- Weight management and physician supervised weight loss.
- Women’s health.
- Men’s health.
- Chronic disease management including but not limited to hypertension, diabetes, thyroid disorders, metabolic syndrome, cardiovascular disease, asthma, COPD, depression, anxiety, erectile dysfunction, chronic anticoagulation management and more.
- Evidence based cancer screening.
- STD screenings.
- Sports and school physicals.
- Coordination for referrals when needed.
- Sick visits include but not limited to upper respiratory illness, sore throat/strep infections, ear infections, pink eye, bronchitis, pneumonia, UTI, STDs, vaginitis, prostatitis, skin infections, shingles, dermatitis, strains/sprains, minor laceration.
- Same day or next day sick visits.
- House calls will be made available through AG Medical Wellness for an additional cost.
- NOTE: We DO NOT routinely prescribe opioids or other narcotics for chronic pain.
Monthly services rates
Individual patient 129/month*
- $100.00 enrollment fee, non refundable administrative charge.
Couples and families
10% discount for each family member, adults and children 18 - 24 years old. 10% discount for yearly payments.
Children 7-17 years old $50.00/month with parent enrollment.
- $150.00 enrollment fee, non refundable administrative charge for couples and families.
Home visits: $200.00 per visit for DPC members only (within 5 miles of practice).
Non-members office visit: $275.00 per visit.
